[EAS] National EAS Test Cable TV Questions

A couple of questions about the National Test in September:

 

Are all cable providers required to carry the National EAS Test? 

 

Are cable companies required to carry the National EAS Test on all channels?

 

Are satellite providers required to carry the National EAS Test on all
channels?

 

What about a really small, community operated provider with less than 50
subscribers? Do they have to carry the National Test on all channels?

 

Have the manufacturers of cable equipment provided upgrades for their
products to include the "000000" FIPS Code and the immediate rebroadcast of
an NPT?
