Bill is right, but I think he left out one important element.  

 

I witnessed very cold water up to the knees of the engineering crew in a
confined space aboard a 399-foot Coast Guard icebreaker, as they worked to
repair a shaft seal issue (the three shafts carry 25K horsepower apiece to
the screws, so this is not a small job).

 

Yes, good procedures and training are vital.  But glues all this together in
an emergency response aboard ship is frequent DRILLS.
