Well, already found a glitch, usual case of someone thinking too much.  We didn't get the test from our WV plan monitoring assignments because the primary relay didn't get the test. Our secondary assignment was too weak to decode (thunderstorm in the area).

But we monitor an Ohio station, since we are on the boundary with Ohio & they often have severe weather alerts for our area ahead of NWS Charleston. 

However, since it was not our "official" assignment listed in Form One, there is no way to enter  it into Form 3.

Have e-mailed ETRS but don't expect an answer.
