Its not needed until its needed. Then everyone wants to know why it wasn't 
working.

With the closing of FCC Field Offices, there are fewer EAS enforcement 
actions anymore. When there has been a lot of media attention, there have 
been a few $1+ million fines (NALs) for ads using EAS tones and the 
investigation of the Hawaii false alert.

There should be a difference between a "safety investigation" and an 
"enforcement investigation."  The FAA and FDA changed from an enforcement 
approach to a safety approach. When the FAA and FDA changed to a safety 
approach, they also increased safety incident self-reporting by industry, 
instead of the catch us if you can enforcement approach.

Telco's already have FCC self-reporting obligations for safety problems 
with 9-1-1 PSAPs and other outages.

I couldn't find a good source for non-weather disaster fatalities from 
wildfires, hazardous material releases, etc.
