[EAS] False EM alarm in HI

I've been reading this entire thread this morning about HI, and have questions.

1. Can someone explain just how the dominoes fell, in what order? (EOC originated, which triggered...?)
2. Do we know for a fact that this false alert crept into the actual HI EAS? I saw the footage of that soccer game and the crawl with the tones, but no duck farts. Was this game on tv or cable?

I take care of the LP-1 here in SE Virginia, where missile strikes are an accepted possibility (Norfolk Naval Station). I'd just like to make sure I'm educated on what happened, so thanks for anything.
