[EAS] Form 3 Deletes IPAWS Entry

I noticed that but slightly different, I got it back as blank in the 
verification, but when I went back into the form via modify the choice 
was still selected.

I guessed that they might have missed putting the 1st source variable in 
print statement. So I just ignored it and kept going. If you do actually 
leave it blank you can't submit the form.
