ETRS Form Three includes about 8 to 10 receiption and transmission quality 
questions and space to report "other" issues.

Form Three could change.  In the last public announcement the Form Three 
questions are

Receiption of NPT

1. Did the facility receive the EAS message? Yes/No
2. From what source(s) did you receive the EAS message? monitoring source 
one, monitoring source two, other monitoring sources, IPAWS
3. Message Receipt Date and Time
4. Time Zone
5. Please indicate if there were any complications in receiving the EAS 
message?
 	No complications
 	Failed to receive alert code
 	Audio quality issues
 	Received duplicate messages (I assume this means duplicates that 
caused problems, not normal de-duplication)
 	Equipment performance issues
 	User error
 	Other
6. Explanation? Free text for anything else

Retransmission of NPT

1. Did the facility retransmit the EAS message? Yes/No
2. Message Retransmission Date and Time?
3. Time Zone?
Please indicate if there were any complications in retrainsmitting the EAS 
message?
 	No complications
 	Failed to retransmit alert code
 	Equipment performance issues
 	User error
 	Other
4. Explanation? Free test for anything else
