[EAS] ETRS Form 2 - What will be asked?

Per FCC, Form Two will only ask two questions:
Did the facility receive the EAS test? Y/N
Did the facility retransmit the EAS Test? Y/N
That's all there is.

Form Three gets into:
>From what source(s) did you receive the EAS message? (checkboxes)
Message receipt date and time. (fill in blank)
Time Zone. (fill in blank)
Please indicate if there were any complications in receiving the EAS message. (checkboxes)
Give explanation of above. (fill in blank)

That's the last I saw, but subject to change, I imagine.

I get questions about what Form 2 will ask for, and couldn't find out in
a cursory google search.  Anybody know?
