When these things happen to me and for no apparent reason, I also

received the next test.
If you only missed one and received it the week before and the week
after and found nothing else wrong or made no changes nor fixed
anything then that's also notable for the report.

I think Clay is saying that you might not be able to pinpoint the
exact cause, but if you show a good effort at trying to figure it out
then you would be on pretty firm ground with the Feds.
