If you are able to cope with a 1RU form-factor Behringer has the SRC2496 - which seems to be stereo digital or analog in to stereo digital or analog out with sample rate conversion up to 96kHz 24bit.

The list price is $299, but street price should be better - eg: Broadcast Supply Worldwide has it currently at $199.

Like a lot of Behringer gear I've used, it's probably too versatile and complex for a simple application, but at a much lower cost than a simpler single-function device.

Sometimes that worked out for me but sometimes it didn't.
All those buttons and knobs to get disturbed....
