I (along with someone else here) once heard a high school station where
the student said "My bad" in the middle of a news story, after incorrectly
pronouncing a word.  This was, sadly, within the past five years.

I also know of another stretch of time - on one of our local TV stations -
where all the 11PM "world news" stories would go to the female co-anchor,
because they had more difficult to pronounce words...and the producers
knew that she never pre-read her stories before going on camera. :-)
