Good question: Many answers. 

An status monitor for both boxes and modern
control COULD by pass the combiner with coax
switches to maintain the correct power (adjusting
the top automatically). ERI, Bird and maybe
Coaxial Dynamics make the sensors. ERI and
Dielectric make OK coax switches. There are other
ways to skin this cat. The mid power combiner can
be a fixed injector or a switchless combiner. The
same sensors and with a good controller can
re-adjust the switchless combiner and TOP. Harris
holds the patten on the mid level combiner, but I
BELIEVE ERI or Dielectric build these devices for
them.
