$2/number/month, and 2.9¢/minute for regular numbers and 3.9¢/minute  
for inbound toll-free.  That's it unless you want features, and they  
aren't much.  The web site is very hard to use, but you CAN find  
stuff after a while.  I have a good friend using them and he is happy  
for his small business.
