I remember a few incidents that aired on WABC-TV/New York.  In one, they had rolled out a birthday cake for one of the news team members, right at the end of the newscast.  As the closing credits were rolling, and the staff gathered around the cake on camera, anchor Roger Grimsby asked one of the camera operators, "You gonna take a close-up?" followed immediately by weatherman Tex Antoine's reply, "He will, if you get out of the f***ing way!"  As you have no doubt guessed, the mikes weren't off.  They sat Tex down at the switchboard for that entire evening to reply to the complaints.

In another incident, a reporter narrating a taped piece in the studio gave one of the floor directors the finger, on camera, while being introduced by another reporter (RealPlayer required).  Roger Grimsby's reaction is priceless:
