Don't know if other states are doing it, but Alaska has a Survey
Monkey page up - linked from ready.alaska.gov - or direct link - and
the survey is quick and simple:
https://www.surveymonkey.com/r/3YNMLT8
Seems like a good way to do it.

The cell companies might report equipment problems or failure-to-relay in some places (IF they were required to report at all, which I don't think they are). But there's so much variance between cell phones themselves that it will be impossible to determine whether a failure to receive was on the part of the cell company or of the phone.
