One factor may be local radio/TV station coverage of the event, which may
have been prolific at the time.
Still I tend to agree. WEA does not get there 100% of the time. People
watching pay-per-view or an internet streaming movie and may essentially be
in an info silo. And if they heard the siren, they may not know how to
react or what it means.
It does appear that police and fire went door-to-door. Leilani Estates is a
population of about 1500. Nanawale Estates is about the same (and that is
in the warning polygon as well).

Maybe it was not a factor in the Hawaii volcano incident, but I am
concerned that there are still emergency managers who do not take to heart
decades of serious warning research.
This research clearly tells the EM community to take advantage of all
available warning systems to get the word out when life safety is at stake.
