Mike,

When I was in charge of EAS logs, I would have a separate form to log all tests. This was reviewed and signed weekly. All the alert printouts from the ENDEC, tests, WX, Amber, etc. were signed and attached to this log.

For what it's worth, this method was acceptable to both our alternate Inspector, and an actual FCC inspection.
