The challenge of the three-ring binder templates, there is a limit how a 
paper form can be structured.

I aggree with your statement, and an on-line user interface could be much 
more dynamic, and should allow the operator to choose the <description> 
template and <instruction> template separately.

There will likely be a few protective action patterns which could be
reused across many different types of hazards.  These could be loaded 
into the <instruction> field by clicking on some standard patterns.

The <description> part would be unique for an incident, or again have a 
different set of standard patterns for various hazards.

I assumed that's why you designed CAP that way.
