I finally received a response from the FCC regarding the missing information on my Form 3 submission.
This was after sending a second email on October 7 to follow up to my first email of September 29.
My original email reads:
Sirs:
I was doing my Form 3 ETRS submission today and noticed an odd thing.
I entered IPAWS using the pull down menu for the source where I first received the NPT.
Then when I move to the next page, where you check the confirmation box that lights up the final green submit button, the IPAWS entry was blank.
I finished submitting, logged out, logged back in and sure enough the answer to the question that ask which source you received first, was blank.
I received the email that said form three was successfully submitted.
I got two emails actually, as I re-submitted Form 3 again.
I also copied and pasted the link provided in that email and verified my Form 3 submission, but the IPAWS entry for the first alert received is still blank.
Is there something I missed or is this a error with the form?
Please advise.
Here is the response I received today.

Dave,
My apologies for the delay in our response. I was tasked recently to assist the staff with the large volume of e-mails.
I'm forwarding your e-mail to tech staff and hopefully you will get a response timely. I see you have Form 3 completed. I'm not sure if the system shows the explanation or not after entered although I agree with you it should, but I can't confirm that as I don't regularly work with this system. Sorry I couldn't help more here. I hope you get a response after I forward this, but I do know we have a few hundred e-mails we are sorting through at the moment. If you need further assistance, please contact me directly. Thanks.
Sincerely,
Al Knerr
