[BC] Streaming providers

Every so often this thread appears I admit.  But the nature of the
business is so dynamic that folks come and go all the time.  Sooo...

Client is looking to change streamers and is looking for professional
distribution providers capable of >50K concurrent connections world wide.
They treat the stream as it's own parallel simulcast station with PPM
encoding. Client does it's own ad insertion though a dedicated traffic'd
automation system so as to maintain PPM coherence. They're only looking
for the true back end distribution/bandwidth. They will mix barter and can
mix some amount of cash. But the barter must be traffic'd through the
station. Downstream insertion isn't part of the equation.

Appreciate comments on who to avoid as well...but please include the
reason.  I don't want a simple case of sour grapes to poison what might
work in this application.
