Look for the possibility that one of the devices connected to the switch has a parasitic voltage on its "ground," i.e., frame. Devices that are powered by wall warts often end up with their lowest potentials elevated by static electricity or leakage. This can break-down a MUX in the switch which is usually not permanently damaging, but can cause them to hang.

The switch "locked up"--no activity indicated on the LED's on the
front panel, network down. Unfortunately, the way the automation is
set-up, the on-air and voice-track machine need to talk to each other
or the whole system freezes up.
