It depends on the state.  Different states have different ways of  
funding everything, from roads, to schools, to fire/EMS, and even law  
enforcement.

Virginia has a lot of state provided funding for local EMS, but there  
is a lot of local control.  The Commonwealth does set (or adopt)  
standards though.
