[BC] EAS Logs

I have stations stapling the tape to the EAS log sheet.  The log sheet is 
something that I got from the San Diego Chapter of the SBE.  I recall that 
it was approved by the FCC inspector there.  I cannot remember where I got 
the log, but I will send it to anyone that wants a copy.
