Goran:
I would say, probably some of each.
Careful listening and the understanding of cause and effect points 
down certain paths.
I think that knowledgeable tinkering then leads one to a series of 
subjective choices.
The designer's "correct" selection from the gamut of available 
choices is what sets designs apart.
